In the event of an accident, injury, or medical emergency during volunteer activities, I authorize Lettermen of the USA representatives to contact emergency medical services when reasonably necessary. This agreement is intended to apply to my participation in all future Lettermen of the USA-sponsored or authorized volunteer events, programs, and activities beginning on the date signed below, unless I revoke my consent in writing. Any revocation will apply prospectively and will not affect activities completed before Lettermen of the USA receives the revocation.*
